Everything safety-related — crisis help, grounding tools, and Carmelita — works without signing in.
Your journal entries are private. This prototype stores them on this device only.
Prototype IDs — clients: RAMELI-3390, DELLUC-4821, REYMAR-5107. Clinicians: CAM-9001, CRI-9004. Non-clinical staff: ADM-9000 (front desk), ADM-9010 (practice manager). Parent: FAM-1001. In the live app each is paired with a password, and staff additionally with multi-factor authentication. Staff access is logged, and signing in to a staff account confirms you are an authorized workforce member on a secured device.
Setting up
Four steps, in order
Each one is optional except the first. You can stop after any of them and come back.
Nothing you write is visible to anyone — not your therapist, not a parent — until you send it, item by item. You can take any of it back later.
Connect
Connect to someone
Enter the Confianza ID of the person you support. They get a request and decide whether to accept it. Until they do, you see nothing at all.
Requests you have sent
What happens next
They see your name and can accept or deny it
If they accept, you also get linked to their provider
You see only what they choose to send you, never their journal
Either of you can unlink at any time, which revokes everything
Step 1 of 4 · New account
Create your account
This takes a minute and gives you a Confianza ID. Everything after this — your therapist, the people who support you — connects to that ID.
Your therapist (you can add this later)
If your therapist has already given you their provider ID, put it in and you will be linked as soon as the account exists. If not, skip it — step 2 comes back to this.
Your full name is used to build your ID and is never shown to anyone but you. Nothing you write is sent to a provider until you send it.

1
Grounding skill
Butterfly hug
Cross your arms, hands on opposite shoulders, and tap slowly — left, right, left. Alternating touch settles a nervous system that is running ahead of you.
left
Tap along with the word. Slow is the point — about one tap a second.
If tapping brings up more than you expected, stop and come back to it with your therapist. That is what sessions are for.
Grounding skill
Stop
Four moves, in order, for the moment before you do the thing you will regret.
S
Grounding skill
Urge surfing
An urge is a wave, not a straight line. It builds, peaks, and drops on its own — usually inside a few minutes — if you stay with it instead of fighting it.
3:00
Press start, then just notice where you feel it and how strong it is.
Riding one urge is not the same as treating what is underneath it. If these come often, bring it to your therapist.
Grounding skill
Step back from a thought
You are not arguing with the thought or proving it wrong. You are putting a little distance between you and it.
Nothing typed here is saved. If you want to keep it, write it in your journal instead.
Grounding skill
Name it to tame it
Putting a precise word to a feeling measurably lowers its intensity. Vague distress is harder to work with than a named one.
Grounding skill
Worry postponement
Worry rarely responds to "stop thinking about it." Scheduling it works better — you're not dismissing it, just deciding when.
1
Grounding skill
Shake it out
Stress mobilises the body for action it never gets to take. Two minutes of deliberate movement gives it somewhere to go, which is why animals shake after a fright.
1
Grounding skill
Hum it out
A long hum makes the out-breath longer than the in-breath and adds vibration in the throat and chest. Both nudge the body toward its rest setting.
2:00
Do it quietly if you are not alone. It works at a volume only you can hear.
Grounding skill
Orienting
Slowly looking around a room tells a vigilant nervous system what it keeps asking: where am I, and is anything here a threat. It is the oldest grounding skill there is.
1
Grounding skill
Name eight
Recall takes up the same attention that worry is using. Pick a category and name eight, out loud or in your head, tapping one dot each time.

Grounding skill
Check the facts
Emotions fit some situations and overshoot others. This walks the gap between what happened and what you concluded, without arguing that the feeling is wrong.
1
If the facts do check out, the feeling fits and the next question is what to do about the situation, not the feeling.
Grounding skill
Both things are true
When a feeling and a fact seem to cancel each other out, the mind picks one and fights the other. Holding both usually settles it faster.
Nothing typed here is saved. If you want to keep it, write it in your journal instead.
Grounding skill
Opposite action
Emotions come with urges. When the urge keeps you stuck, doing the opposite — gently — tends to shift the feeling faster than waiting it out.
This is for emotions that are out of proportion or keeping you stuck — not for feelings that fit the situation. Sadness after a real loss doesn't need opposing.
Grounding skill
Self-compassion break
Three steps, about two minutes. Useful when the voice in your head has turned harsh.

Everything in this app is open to you already. Linking your account to your provider adds one thing: the ability to send what you write to your therapist or a parent.
What linking adds
Send your journal, mood, and goal history to your therapist

Give a parent or guardian their own limited sign-in
See exactly who opened what, and take any of it back
Your connection code comes from the office when you start services.

Privacy & security
How your information is handled
Confianza Health is a covered entity under HIPAA. Anything you send your provider becomes part of your clinical record and is protected by the same rules as anything you say in session.
What this app does
Nothing is sent to your provider unless you send it
You choose the exact items and date range — the minimum necessary
You sign an authorization before anything is released
You can revoke a share, which removes your provider's access
Every open, download, and send is logged and shown to you
Providers see only clients linked to them
The portal locks itself after 5 minutes of inactivity
Regular email is off unless you specifically allow it
Parent & guardian access
A parent or guardian sees only what you send them, never the app itself
What you send a parent is separate from what you send your therapist
Your therapist does not see what you sent your family, and vice versa
Every time a parent opens something, it shows in your access history
Removing someone's access revokes everything you sent them
Parenting tools and crisis resources are free with or without a code
If you are a minor, what a parent or guardian is entitled to see is also set by Arizona law and by Confianza Health policy, not only by this app. Your therapist can walk you through how it applies to you. Questions about your record: (623) 439-7472 or info@confianzahealth.org
Assessments
Screening instruments are administered by a clinician, not self-completed
A score on its own is not a diagnosis and is not meant to stand alone
Results are recorded against the provider's portal session
Your rights
You may see and get a copy of your record
You may ask for corrections to it
You may ask for a list of disclosures
You may revoke an authorization in writing at any time
Revoking does not undo what was already relied on
You may file a complaint without any effect on your care
This build is a prototype, not a compliant system
It stores everything unencrypted in this browser, has no real authentication, and is not covered by a business associate agreement. Do not enter real client information. See the production checklist below before this is used with anyone's actual health information.
Before go-live
HIPAA-eligible hosting with a signed BAA
Server-side storage, encrypted at rest and in transit
Named accounts, MFA, role-based access, automatic lockout
Server-side immutable audit log with retention
Routing shares into the EHR rather than a separate store
Signed authorization retained per state and HIPAA rules
Breach detection, notification procedure, and incident log
Security risk analysis and workforce training on file
Device-level protections: no PHI cached, remote wipe

Practice administration
Restricted to authorized practice staff. Every notice you send is signed with your name and kept in the audit trail.
This is not clinical messaging
Notices reach many people at once and are not read by a clinician. Never put protected health information in one, and never use one to respond to a crisis.
Prototype codes: ADM-9000 (Front desk), ADM-9010 (Practice manager). Production requires named accounts with multi-factor authentication.

Screening tool
This is a screening tool, not a diagnosis. Results indicate whether symptoms are consistent with a pattern — only a licensed clinician can diagnose.
Results are not a substitute for a clinical evaluation and should not be used alone to make treatment decisions.
These instruments are intended for use by licensed clinicians as part of a full assessment.
A low score does not rule out a condition, and a high score does not confirm one.
If you or someone else is in crisis, stop and use the crisis resources in this app, or call 988 or 911.
